The NEOS Unified eMailing includes electronic versions of the NEOS Unified Mailing packet that can be viewed and printed from an Internet browser. There are times when you may need a copy of a document in its original format. Additionally, copyrighted documents, confidential documents and items from other non-NEOS organizations may not be included in the eMailing due to legal constraints. You can use this form to request copies of these documents in their original format. The NEOS office staff will process your request in a timely manner and drop your request in the mail.
